Douglas Private Care Intake and Assessment Team works together to:

• Conduct eligibility screenings for people seeking developmental disability services
• Schedule and conduct assessments and annual updates for people receiving services and produce a summary of findings for each assessment completed
• Participate in the development of Individual Support Plans by suggesting appropriate services, supports, and other recommendations
• Ensure continued eligibility for services through level-of-care reviews and approvals
• Provide pre-approved consultation, technical assistance, and training to providers and staff to effect best practices in service delivery and implementation of ISPs
• Assure that consumers are afforded all due process and appeal rights
• Maintain documentation and tracking of all screening/evaluation functions
